If this is your very first appointment with us, please arrive 15 minutes before your scheduled appointment. This will allow us to finish any paperwork and stay on schedule with your appointment.
Your first appointment duration varies, but please plan on 1 to 1.5 hours.
Please bring the following items with you to your first appointment:
• Dental Insurance Card (If applicable)
• Identification (Driver’s license, military ID or state ID)
• Forms
• Patient Information/History
• Informed Consent
If you require pre-medication before your appointment, please be sure to request a prescription prior to your appointment, or if you are unsure, contact us and we can help.
We accept most insurance plans and are more than happy to process your claim. Please bring your insurance information with you to your first appointment.

For services not covered or not submitted to insurance, we accept cash, check or major credit cards. We do ask for payment at the time of service.

Now you can join our low-cost dental membership for a nominal membership fee. Our plan entitles you to preventive dental care at no cost! Corrective services are available for small co-payments that are far less than the usual, customary fees. Our professional staff is qualified to care for all of your dental needs!
